MANAGING YOUR ACCOUNT
Your HSA Insurance Card - The first thing that you should do when you visit your physician or pharmacist is to present your USHL insurance (ID) card.  This will ensure that you receive the care you need without the need for phone calls or confusion.  Your HSA insurance card ensures that:

  • Your pricing is set - Your card ensures that you get the discounts or pricing that US Health and Life has negotiated for you in advance. 


  • Administrative services provided:
    • Managing your deductible - As part of your coverage, we will insure that you receive deductible credit for covered medical expenses paid from your HSA. 
    • Timely payment - Once you've met your deductible, USHL will make sure that your provider or pharmacist is paid in a timely manner, so that you won't have to worry about it.
    • Keeping you informed - After you receive services, USHL will send you an Explanation of Benefits (EOB) that shows you how your bill was processed.  You can then compare the EOB to the bill that you received from your provider or physician to ensure that you're not over-paying or getting charged for services that you did not receive.

Your HSA Debit Card - Your USHL debit card is a convenient way to quickly access the funds in your HSA account, with no hassle, at hundreds of thousands of locations worldwide.  It looks, feels, and functions almost exactly like any debit card that you already have.  Most hospitals, clinics and pharmacies in your network will accept your debit card for payment, but for the few who will not, you can use a check (a starter set is provided when you open the account).  
Access to HSA Account Information –  You will have access to your account information with:

  • Monthly statements -  Mailed to you at your home with all the information you would expect to receive from a monthly statement.  The statements are an excellent additional source of documentation for your files.
  • Toll-free phone support – Just dial 1-866-841-0370 and you can either use the interactive system to listen to your account balance, recent deposits, and withdrawals, or speak to a representative who will answer any questions you might have about using your HSA.
  • On-line tools – At www.umb.com, you can check your account balance and track the expenditures you make with your checks or debit card.  This service is available 24 hours a day, 365 days a year, from anywhere in the world, with internet access.
  • Not a credit card – The US Health & Life HSA debit card is not a credit card.  You will not pay interest on any eligible expenses that you make, but your spending is limited to the amount of money in your account.  Be sure to utilize the information tools to avoid problems or fees.

Care Management - USHL is constantly looking for new ways to help members improve their overall health, manage disease, and improve their lifestyle.  This page contains links to organizations and companies that can assist you in managing your total healthcare through healthy living, prescription assistance drug programs, patient safety, hospital comparisons, and other helpful information, such as:

  • Disease or condition specific information - To assist members in their effort to improve their overall health, manage disease, and improve their lifestyle, links to organizations and companies can assist you.  For example, The American Cancer Society, Association of Cancer On-line Resources, American Diabetes Association, American Heart Association, American Lung Association, Fertility Friend, and Your Disease Risk
  • Prescription assistance program – Provides information on organizations that can assist you in obtaining medications at low or no cost if you meet certain qualifications.   For example, HelpingPatients.Org, Needymeds.com, RX Outreach, RX Hope, and Free Medicine Program
  • Providers and facilities – Includes information about patient safety and hospital comparisons through links to other websites, such as The Health Pages, Department of Health and Human Services-Hospital Comparisons, 15 Steps That You Can Take To Reduce The Risk Of Infection In The Hospital, The Leapfrog Group, Consumer’s Checkbook, and JCAHO. 
General health & wellness - Includes links to organizations that provide tools and risk assessments to maximize your health.  For example, National Institutes of Health, American Public Health Association, Coverage Options Database, DR. Green – Caring for the Next Generation – Pediatric Care, Healthweb.org, and Medline Plus.
