census (employee name, gender, birth date, dependent status,
income and job title)
• Existing Benefit Design (booklet summarizing benefits for each
plan)
• Copies of the most recent billing statements
• Benefit budget allocation
• Benefit objectives
• Pre-existing health information
• Identifying out-of-state employees
• COBRA beneficiaries
